Center Stage (@centerstagepage) is a music diplomacy initiative that brings international performing artists to the United States to encourage cross-cultural understanding and collaborations. Along with Argentina and Armenia, Taiwan has the prestige of being chosen to participate in the Sixth Season of Center Stage. Seven ensembles selected from the three regions will travel to the United States for a month-long tour during 2022-2023. This program is fully funded by the U.S. Department of State (@ECAatState) and the New England Foundation for the Arts (@NEFApage)! เชื่อว่าหลายคนอาจทราบว่าขณะนี้เรื่องนี้กำลังก่อให้เกิดปัญหาในการใช้ยาคุมกำเนิดเพื่อคุมกำเนิดและการรักษาโรคเป็นอย่างมากในประเทศไทย
เนื่องจากมีข่าวและแชร์ออกไปอย่างกว้างขวางทำให้มีความเข้าใจที่อาจคลาดเคลื่อนไป และไปเข้าใจว่ายาคุมอันตรายมากมาก
คนไข้จำนวนมากที่แพทย์ได้ตรวจพบโรคหรืออาการบางอย่างที่ควรจะต้องรักษาด้วยยาคุมกำเนิดชนิดทาน มาโรงพยาบาลเพื่อ “ขอหยุดยา”

กินยาคุมแล้วเสี่ยงเป็นมะเร็งเต้านม...จริงหรือ???
อันที่จริงเรื่องนี้มักเป็นประเด็นให้ถกเถียงกันอยู่บ่อยๆ และเป็นสิ่งที่คนไข้รวมไปถึงญาติๆส่วนใหญ่มักจะถามเวลาที่หมอสั่งยาคุมกำเนิด ไม่ว่าจะเป็นการใช้ยาคุมกำเนิดเพื่อคุมกำเนิดหรือเพื่อรักษาโรคก็ตาม
#กินยาคุมแล้วจะเป็นมะเร็งเต้านมไหมคะ???
วันนี้หมอจึงนำข้อมูลการศึกษาที่ตีพิมพ์ใน New England Journal of Medicine ปี 2017 ซึ่งเป็นวารสารทางวิชาการชั้นนำที่เชื่อถือได้มาเล่าให้ฟังกันค่ะ
เขาทำการศึกษาในคนไข้ชาวเดนมาร์ก 1.8 ล้านคน ซึ่งจำนวนคนในงานวิจัยเยอะขนาดนี้ถือว่าทำได้ยากมากและผลที่ได้จะน่าเชื่อถือมากทีเดียว คนเหล่านี้ใช้ยาคุมกำเนิดช่วงอายุระหว่าง15-49 ปี ติดตามดูการเกิดมะเร็งเต้านมเป็นระยะเวลา 10 ปี พบว่า
- ไม่เพิ่มความเสี่ยงของมะเร็งเต้านมเลย ถ้าหากใช้ยาคุมกำเนิดน้อยกว่า 1 ปี
- เพิ่มความเสี่ยงเพียงเล็กน้อย...”ย้ำว่าเล็กน้อยเท่านั้นนะคะ”... !!! หากใช้ยาคุมกำเนิดนานขึ้น โดยเมื่อคิดเปรียบเทียบกับความเสี่ยงของการเกิดมะเร็งเต้านมในคนที่ไม่ได้ใช้ยาคุมกำเนิด พบว่า ในคนไข้ทุกๆ 7,690 ราย ที่ใช้ยาคุมกำเนิด...จะเพิ่มความเสี่ยงของการเกิดมะเร็งเต้านมเพียง 1 รายเท่านั้น
- เมื่อพิจารณาแยกตามชนิดของฮอร์โมนที่เป็นส่วนประกอบในยาคุมกำเนิดแล้วพบว่า ความเสี่ยงของการเกิดมะเร็งเต้านมจะเพิ่มขึ้นในกรณีใช้ยาคุมกำเนิดรุ่นเก่าๆและที่สำคัญเป็น highlight เลยคือหากเป็นฮอร์โมนรุ่นใหม่ๆพบว่าแทบจะไม่ได้เพิ่มความเสี่ยงของการเกิดมะเร็งเต้านมเลย
- หลังจากหยุดยาแล้ว ความเสี่ยงนี้จะลดลงเท่าคนปกติได้ หากใช้ยาคุมกำเนิดเป็นระยะเวลาไม่เกิน 5 ปี
ในทางกลับกันค่ะ...การใช้ยาคุมกำเนิดในการรักษาโรคบางโรค เช่น PCOS ซึ่งเป็นโรคที่มีภาวะไข่ไม่ตกเรื้อรัง ซึ่งจะเพิ่มความเสี่ยงในการเกิดเยื่อบุโพรงมดลูกหนาผิดปกติ และเสี่ยงต่อการเกิดมะเร็งในโพรงมดลูกได้มากขึ้นถึง 3 เท่านั้น ทราบกันใช่ไหมคะว่าการใช้ยาคุมกำเนิดจะ “ช่วยลดความเสี่ยง” นี้ได้ ไม่เพียงเท่านั้นนะคะ มีการศึกษามาแล้วว่าการใช้ยาคุมกำเนิดนอกจากจะช่วยลดการเกิดมะเร็งโพรงมดลูกได้แล้ว ยังช่วยลดการเกิดมะเร็งรังไข่ได้อีกด้วยค่ะ
ดังนั้น หากแพทย์สั่งให้ใช้ยาคุมกำเนิดเพื่อรักษาโรคควร “ต้องทานต่อ” “ไม่ควรหยุดยาเอง” และแนะนำให้ตรวจเต้านมด้วยตนเอง รวมถึงเข้ารับการตรวจแมมโมแกรมตามคำแนะนำของแพทย์ด้วยค่ะ

กินยาคุมกำเนิดแล้วมีก้อนที่ตับ
เจ้าก้อนที่ตับที่หลายคนกล่าวถึงมีชื่อเรียกว่า Hepatocellular adenoma เป็นเนื้องอกธรรมดา (Benign tumor) ไม่ใช้เนื้อร้ายค่ะ
ความชุกของการเกิดโรคไม่ชัดเจนนัก
แต่ก็พบได้ประมาณแค่ 1-4 คนใน 1000 คนในกลุ่มประชากรทั่วไป
และหากมองแค่กลุ่มสตรีแล้วพบว่าความชุกของโรคเพียง 3-4 คนใน 100000 คนเท่านั้น
จึงเป็นเนื้องอกตับธรรมดาที่พบได้น้อย ทำให้ข้อมูลเกี่ยวกับเรื่องนี้มีอย่างจำกัด
แต่จากการเก็บรวบรวมข้อมูลพบว่ามีความสัมพันธ์กันการใช้ยาคุมกำเนิดชนิดกิน โดยขึ้นกับปริมาณของฮอร์โมนเอสโตรเจน และระยะเวลาการใช้ที่นาน
โดยพบว่าหากระยะเวลาการใช้มากกว่า 2 ปีจะพบความเสี่ยงที่เพิ่มขึ้นของก้อนเนื้อตับชนิดนี้ อย่างไรก็ตามเป็นการเก็บข้อมูลในช่วงปี 1975-2008 ซึ่งในขณะนั้นยาคุมกำเนิดมีปริมาณของฮอร์โมนเอสโตรเจนที่มีปริมาณสูง ต่างจากยาคุมที่ใช้ในปัจจุบันที่มีปริมาณฮอร์โมนเอสโตรเจนต่ำลงมากจึงทำให้เกิดความปลอดภัยมากขึ้นแล้วในปัจจุบัน นอกจากนี้หากหยุดกินยาคุมกำเนิดแล้ว ก้อนก็สามารถเล็กลงได้
ตามความเห็นของผู้เขียน ยาคุมกำเนิดยังมีประโยชน์มากกว่าโทษ ไม่ว่าจะเป็นการใช้คุมกำเนิดอย่างมีประสิทธิภาพ
