之前有人問我:「王醫師,斷食不是會啟動什麼細胞自噬作用嗎?可以清除廢物?」
親愛的孩子,你知道嗎,我們中研院為了找出能夠 #終止「細胞自噬」現象,抑制癌症或糖尿病等造成的肌肉萎縮,發現了關鍵因子「KLHL20」蛋白,希望能成為治療的方向。這項研究2015年12月10日登上國際期刊「分子細胞(Molecular Cell)」。
「細胞自噬」能讓細胞維持在平衡狀態,過多或過少都會產生問題。
自噬過少的話,會使有害物質一直堆在細胞內沒錯,但細胞自噬過多,會導致細胞死亡,例如:發生在肌肉細胞就會造成肌肉萎縮。
有科學家努力的想要找出終止「自噬現象」的調控蛋白,你卻千方百計讓身體刻意發生自噬?
這個問題很像之前有病人問我:「在高山上腦部缺氧時,會不會啟動身體的修復機制活化大腦⋯⋯」我的回答是,這是一個臨界點,有可能被活化,但有沒有想過也可能就這麼腦死?
施打葡萄糖到組織的增生理論,也是破壞造成免疫反應啟動,促進膠原蛋白增生,但是「增生」跟「纖維化」的差別可能就是一個臨界,你怎麼知道發炎後活化的路徑是再生還是纖維化?
2017年有一篇文獻說間歇性斷食(12-16小時)會啟動自噬而消除腦內異常蛋白降低失智風險,我同樣想問的是,怎麼知道異常蛋白被消除前會不會肌肉先掉了大半?
「自噬反應」就跟減脂一樣,全身影響,並沒有辦法指定自噬掉什麼,就像沒辦法指定只瘦肚子不瘦胸,跟個人先天體質和後天基因有複雜的關係。
過多過少很難拿捏,臨界點在哪裡,往往也只有神知道。在沒有動物實驗證實在哪個臨界點是「沒有自噬掉好的組織的情況下,只自噬掉廢物」的參考依據,我不會貿然建議任何人嘗試斷食。
就像挺過了變成美國隊長,挺不過變實驗犧牲者。

肺癌傳統被認為與吸菸畫上等號,不過,臺灣的不吸菸肺癌患者卻比吸菸者多。中央研究院化學研究所陳玉如所長、中研院楊泮池院士與跨單位研究團隊以蛋白基因體技術建立臺灣早期肺癌病人的多體學大數據,找到不吸菸肺癌患者可能的致病機制。研究主要有三個發現,首先是肺癌和人體體內APOBEC突變特徵的高低、其次與致癌物的曝露有關;最後更發現一個從未被發現的新亞型肺癌,及其致癌基因突變的差異,有助早期發現臨床潛在的高風險肺癌患者。
研究主持人陳玉如表示,「這是東亞第一套結合深度多體學大數據及完整臨床資料,深度解析不吸菸肺癌成因的研究成果」,也是美國臨床蛋白基因體學腫瘤分析聯盟(CPTAC)首次和國際聯盟團隊(臺灣)攜手合作,以蛋白基因體學揭開臺灣及美國病人肺癌生物學的面紗。臺美二個獨立成果於今(2020)年7月9日同步發表在國際頂尖期刊《細胞》《Cell》上,並共同榮登本期雜誌封面。
