IMG (international medical graduate, 外國醫學系畢業生)的報名人數比過去五年都低,但錄取率(match rate)卻提高不少來到了56% (我去年才52%啊…),照這樣下去說不定哪天會突破6成。由於川普政府的移民政策緊縮,導致許多國家的醫生不想(或是不敢)來美國行醫。畢竟準備個USMLE加上來美國面試的錢對許多貧窮國家的人來講是很大膽的投資 (至少要花30萬台幣加上1~2年的時間,詳情請看 「台灣囝仔」成為「美國醫師」之路,淚與汗與金錢堆疊出的超級擂台) 去年川普使用手段,在match 放榜之前突然來個travel ban,讓program不敢錄取這七個國家來的醫師,多年努力瞬間化成泡影,真的很不公平。
然而我認為現在對台灣的醫學生來講,反而是個逢低進場的好時機。美國每年開放的resident position越來越多,雖然他們拼命蓋medical school (尤其是DO school),但醫師的缺口還是必須要靠外國醫師補足。現在競爭者少了,對於像台灣或是日本這些川普政府相對友善的國家來的申請者就match就變簡單了。我的移民律師跟我說台灣在全世界的國家中美簽通過率是非常高的。她有很多中國籍的客戶,最近學生簽證申請10件有10件下不來,台灣通過率卻一直維持9成以上(她很感慨的說台灣真的要努力維持我們的經濟實力跟人文素養,否則哪天我們的優勢恐怕會消失….)
壞消息是今年因為Trump政府故意刁難的關係,很多IMG的H1簽證無法像往年一樣在七月一日上班日前下來,也因此很多人1. 被迫改成 J1 2. 取消合約。輕者則因為無法如期上班而被用vacation的名義等待,白白損失了一個vacation…R1沒有vacation很難熬啊…不過能拿到H1 visa還是很令人高興的事情! 明年就不知道了….很多program(包括我們家)的program director都在考慮是不是要取消H1 visa,否則醫院人力開天窗真的很困擾。因此台灣人來美國行醫拿H1 visa的機會會越來越少……雖然J1 visa不如H1 visa方便,還需要做waiver, 但對想作fellowship的人來講也不見得是壞事,畢竟提供J1的fellowship比H1多很多,比較容易錄取。
我鼓勵台灣的醫學生趁這個機會認真考慮來美國接受住院醫師的訓練。我個人很喜歡這裡的訓練風格 ( 詳情請看 「大家都累了」──與台灣的「功德醫院」相比,美國的基層醫護們為什麼可以享有「佛系值班」的待遇?)還有主治醫師薪水也比台灣高不少喔!

PS 當然來到這裡之後很想念台灣或是覺得台灣的訓練比較好的醫師也是有,見仁見智。時常覺得寂寞也是必然的。但我覺得【出走】有很大一部分也是想多看看這個世界,離開舒適圈成長真的很快!
