The plot is centered around a woman named Sarah who regains consciousness in an abandoned hospital with no memory of how she got there. A mysterious voice tells her to find the secrets of the hospital. With a cell phone, which is her map, she must explore the haunted hospital and its criminal past in order to escape. As she explores the hospital and the adjacent prison, Sarah begins to learn of the hospital's past, where twelve witches were executed and buried on the hospital grounds, with a thirteenth witch escaping death due to her turning in the other twelve witches. After gathering all of the clues, Sarah realizes that she is the descendant of the thirteenth witch, which is why the spirits of the other twelve witches are haunting her.

Point Man和Paxton Fettel，這兩位角色的身份是兄弟，都是前作中的紅衣女艾瑪（Alma）所生，並且還是敵對的立場。Point Man是前代的主角，一名超級士兵，受命要殺死Paxton Fettel，後者則具有精神感應能力、並以此屠殺無辜大眾。雖然兩者之間有著血緣關係，但Point Man最後仍槍殺了Paxton Fettel。不過到了三代，事情卻產生了一些變化。

Nine months after the events of both games,the Point Man was captured by Armacham soldiers and interrogated at a prison in Brazil, South America. Paxton Fettel, now a "Spectre", interrupts the interrogation and helps free the Point Man. The two form an uneasy alliance and escape the prison through the slums. During their escape, the brothers discover that they - and their tortured mother, Alma Wade - are being hunted by "the Creep", a monster unintentionally formed from the cruel memories of Harlan Wade.
