疫苗,有得打,就趕快去打,還沒輪到你,就請繼續防疫生活。
每一劑疫苗都得來不易。
一般來說,一劑疫苗量是0.5ml,一瓶疫苗大約有 5.5ml 的藥液,所以一瓶,平均可以抽出10劑,給10個人注射。
今天有一個新聞,新竹施打率打了109.22%。
竹北市衛生所,剛才更發佈,他們也超打了。竹北的數據,1790劑的疫苗,打了1955人。
有人開玩笑說這是客家精神,其實不是。
其實,全台灣都在「超量施打」。
這個原因到底是什麼?
因為,台灣人,很努力。
所以台灣的醫護人員,任何一點overfill與residual volume都不放過。
每一瓶疫苗的在工廠時,要把藥液,透過機器分裝到藥瓶裡,為了保留施打時的偏差值,會在分裝時,多加一點點進去。
這叫做「overfill」。
Overfill是每一瓶藥,稍為多出一兩滴的藥液。
Residual Volume是,殘留在藥瓶、針頭、不小心漏掉的任何殘存,一般來說會被浪費掉的藥液。
所以,一個正常的施打,一瓶就是打10個人。
但如果施打的護理師,是非常小心謹慎在抽出每一劑藥液,一滴也不浪費,甚至抽完後,還把殘餘在藥瓶角落的滴劑,也仔細抽取出來,那是可能可以抽出更多,也可以施打更多人的。
日前,在莫德納疫苗抵達台灣時,我在加拿大溫哥華的Covid專責病房服務的藥學博士朋友 Hans Lin,就針對此事對台灣衛生單位提出建言。
林博士說,根據他們過去半年來,臨床注射莫德納疫苗的經驗,如果使用得當,巧手抽取,一瓶可以從10劑,增加為11劑,在幸運的時候,還可以抽出12劑。
疫苗不夠,這是全球皆有的現象,因此,就算過去注射很豪邁的美國、加拿大的醫護人員,也都開始學會省著用,希望一瓶能抽出更多劑量,還因此開發出自己的方針。BC省的疾管中心(BC CDC),甚至直接把這種抽取方法,列為官方注射建議方法。
我也依此與我們基隆第一線的醫護人員詢問,台灣目前有的AZ疫苗、以及接下來要開始施打的莫德納疫苗,其實大家在每一瓶、每一劑的抽取上,都是這樣,費盡力氣,就是希望多抽一點藥液出來。
要知道,光是一天要穿著厚重、濕熱隔離防護衣、護目鏡、口罩,已經非常不舒服,在此時此刻,還要付諸高度專注力,小心翼翼確保抽取藥液的每一小細節都不放過。
多抽藥液,其實都是增加工作的壓力、強度。
有的人工作時,可以打混摸魚,也有的人,工作多到一整天都在忙,忙完了天都黑了的經驗。
我們現在第一線的醫護人員,就是隨時隨地,都是這種高壓、高強度的工作狀態。
我們該做的,除了對他們表示謝意、敬意,並且在未來若要討論起當前健保制度的缺失、醫護人員待遇太差、工時過高等問題時,大家都還記得這一群人現在如何保護著這個國家、照顧著我們全體國民,理性客觀長遠地來討論這些問題。
少部分人,在最近,竟然還對醫護人員暴力相向,或是稍微有點不便,就抱怨不停的行徑,這實在讓人無法容忍。
今天林右昌市長也有提醒大家,有輪到你施打,就去施打。有經過政府檢驗、通過的疫苗,每多施打一個人,都是增加社會的保護力。

如何抽出第11劑的COVID19疫苗藥液?
After preparation of your expected tenth dose, there will be serum that is potentially clinging to the interior of the vial, those droplets can be what’s the difference between you and your eleventh dose.
當你抽完第10劑藥液後,藥瓶裡還會有最後一點殘餘藥液,殘存在藥瓶內壁,這些藥滴將是你有沒有辦法抽出第11劑藥液的關鍵。
Inverting the vial will allow for that to settle down into the bottom of the neck for you to retrieve.
把藥瓶倒過來放,讓殘存藥液流到瓶頸,才能抽取。
We found that if you have the time, please allow this to sit for nearly 20 minutes before trying to prepare your eleventh dose.
我們發現,假使你有時間,可以倒置藥瓶20分鐘,再抽取。
If you don’t have time to allow for 20 minutes for this vial to sit, some manual manipulation may help got those additional drops down to the neck of your vial.
如果你沒有20分鐘可以等,可以手動去引導藥液流到瓶頸。
Once ready to prepare your eleventh dose, make sure that the vials remain inverted and go ahead and reclean that surface to ensure that no contamination has occured.
當準備好抽取你的第11劑時,確保藥瓶是保持倒著的,瓶塞表面再清潔一次,避免污染。
Inject your needle at a 45 degree angle to allow for it to rest in the neck of the bottle where groove meets the neck.
將針頭以45度插入藥瓶中,針溝置於瓶頸旁接觸瓶頸。
If you add a little additional volume of air to the vial it helps with that suction, draw at that angle to remove the last droplet possible also note that below where the eye level can see, there may still be some additional serum below the rubber stamp.
So you may also need to draw from within there. Then ensure that you have your full eleventh dose at the full 0.5 ml by utilizing patients in this method, we have been able to receive a majority of 11th doses.
如果你對藥瓶注入一點空氣,更有助於抽取,抽針時保持同個角度,盡量把最後一滴也抽出。同時注意,在橡皮瓶塞下方,視線看不到的角落,還有最後幾滴。所以你可以從那裡抽針。這樣你就可以抽取到你的完整的第11劑疫苗(0.5ml),更能衝高現有疫苗可施打的人數。我們多數都能從一瓶裡抽出11劑。

This video is about Gina’s fitness arm routine FOR BICEPS AND TRICEPS. Please note Gina definitely likes to do other exercises as well but these are her FAVES!
As we say in the video, you just have to find what works for you! For us, we have learned so much from being in the gym and working with trainers (we are still learning), and we hope that from this video you can learn something too and achieve those lean cut arms!
There are 6 exercises in total (3 for biceps and 3 for triceps) to complete this arm workout routine. Feel free to follow along below, and try these exercises out for yourself. Please do what works for you and of course at anytime if something doesn’t feel right or causes you pain please stop! Always listen to your body :)
1. BICEP HAMMER CURL
-Choose a weight that is heavy enough for resistance
-Stand hips width apart
-Hold the weights with your thumbs facing upward, keep your elbows at your side
-Lift straight up and down x10 times
-Do 3 repetitions of 10
(For Level 2 lift one foot onto your toes and do the same steps described above. Alternate each side. This will also work your core while working your biceps)
2. BICEP STRAIGHT UP CURL
-Choose a weight that is heavy enough for resistance
-Stand hips width apart
-Hold the weights with your palms facing upward, keep your elbows at your side
-Lift straight up and down x10 times
-Do 3 repetitions of 10
(For level 2 lift one foot onto your toes and do the same steps described above. Alternate each side. This will also work your core while working your biceps)
3. BICEP STRAIGHT BAR CURL
-Choose a weight that is heavy enough for resistance
-Stand hips width apart and slightly bend your knees
-Light straight up and down x10 times
-Add your breath with a strong exhale as you lift up
-Do 3 repetitions of 10
4. TRICEP KICKBACKS
-Choose a weight that is heavy enough for resistance
-Hold the weights with your thumbs face up
-Stand hips width apart, lean forward so your back is at a 45 degree angle, and slightly bend your knees
-Keep your elbows at your sides at a 90 degree angle and push the weights back x10 times
-After 10 times keep the weights behind you, turn your palms up and lightly pump for another x10 times
-Do 3 repetitions of 10
5. TRICEP PULLDOWNS
-Choose a weight that is heavy enough for resistance
-Stand hips width apart and slightly bend your knees
-Keep your elbow at your side and pulldown the rope separating at our hips
-Do 3 repetitions of 10
6. TRICEP DIPS
-Grip the edge of the bench or chair with your hands
-Step away for the bench or chair and bend you knees at a 90 degree angle
-Lift our butt off the bench or chair and dip down while keeping your elbows pointing upward and close to your sides
-Do 3 repetitions of 10
(For level 2 extend the legs straight out and follow the same steps as described above)

This isolation exercise builds a bigger & stronger upper part of chest muscle.
1) Set up an incline bench to an angle between 30-45 degree
2) Lay back on bench, straighten your arms while holding Dumbbell with neutral grip. Ensure feet stays planted to the ground & bend your elbow slightly to assume starting position.
3) Lower the dumbbells to the sides slowly, keeping arms slightly bent.
4) Squeeze the chest muscle & lift the dumbbells up using the same path as you did on the way down.

chinese new year is all about meeting up with my long lost relatives, and we all know that elders don't like heavy make up. so i'm going light on my CNY make up, and try to brighten it up as much as possible!
i'm still using the brown palette for this look as it gives me the most natural look. it's a variation of the previous video's make up : i used my canmake palette as well (because there's the gold there!). i layer the gold eyeshadow to add bling blings on to my eyes.

and i've included the part where i curl my eyelashes. i usually leave it out because i was told that it looks scary when i curl them. i'm just giving it a small tug at 45 degree angle to curl them better! so that's my trick. and it's probably good for you to invest in soft and spongy eyelash curler rubber, because they curl lashes better. the new ones i got are hard and it takes me much more effort to curl my lashes. : (
OH BTW! i'm wearing my new GEO lenses in this video! totally love this one because it looks SOOOO natural! it's FC-724 (fresh brown). probably going to re-order this and leave out the fresh grey.
