When this cute patient requested for veneers, I asked her: “Are you sure…?” After asking her to reconsider and a follow-up appointment few weeks after, we decided to proceed with veneers and she couldn’t be happier with her new smile! 🤗

In cases like this, it is important to communicate and understand the patient’s concern since the aesthetic of her dentition is quite acceptable.
After addressing her concern (Incisal embrasures & Incisal edge morpho), as well as staying as conservative as possible with composite veneers, she ended up with a bright happy smile! 😊

Gotta be honest, this is one of the most challenging cases I’ve done so far. Single anterior discoloured tooth is always hard to deal with, especially with a gummy smile. But I’m excited to share!🤗
To achieve this result:
Smile Design • Gingivectomy • In-office Dental Whitening & Internal Bleaching • Mock-Up • 7 unit Composite Veneers & 1 unit Ceramic Crown
Patient has excessive gingival display (“gummy smile”) with hyperactive upper lip elevator muscle. We’ve discussed the options of:
🦷Orthodontic TX with TADs (“bone screw”)
🦷Botox injection to help reduce the upper lip elevation upon smiling
🦷…or simply accepting the profile and patient just have to learn to smile a little less wide
Patient opt for no Ortho tx nor Botox, and only the treatments I’ve mentioned above. Fully understanding the limitations of the improvement we can bring her, she trained herself to smile to her ideal height and I actually think it looks good.😊Sometimes, no treatment is also a form of treatment, don’t you agree?
Composite veneers weren’t too hard to place. However, it was difficult to mask the discoloured tooth. Root canal treatment was done many years ago and we can actually see the root discolouration showing through the gingiva. (Watch my story to learn more) Even with internal bleaching, the improvement was limited, unless we carry out soft tissue surgery to increase the thickness of the 21 buccal gingiva to mask the root discolouration.
We managed to mask some of the crown discolouration on 21 with procelain-fused-zirconia crown (to match the crown to the rest of the tooth was difficult as well) and the help of a little opaquer, yet there is still room for improvement in my opinion.
Although challenging, I’m blessed to have a very kind patient who’s willing to go through the whole treatment with me. There’s no difficult treatments, as long as our partner (aka patient) is a great one. Of course, it is cases like this which help us become better clinicians. Happy Independence Day Malaysia! 🇲🇾✨
Let’s take a look at this case where the central incisors decided to be “independent” of each other….Yeap, we’re talking about Median Diastema [spacing between your front teeth].🤗
Why not Braces? 🤔 Answer’s in the second part of this caption.☺️But first, let’s walk through the treatment. 🦷
To achieve this result:
Composite Restoration removal • Smile Design • In-office Dental Whitening • Mock-Up • 8 unit Composite Veneers
Patient initially came in with old, overhanging composite filling on her centrals (swipe left to see). Note that how the overhanging margins have caused Gingivitis!
We discussed about the treatment option of getting Orthodontics treatment (“braces”) to close the gap after filling removal, but before that, we have to consider 2 things:
🦷 Width of the Central Incisors and the Laterals might not be of the proper proportion after filling removal. IPR may be required on the laterals to balance out the ratio.
🦷Patient still has a Class III occlusion (lower teeth are in a more “frontward” position) despite having Braces done before. We might need lower IPR/premolar extractions/retraction with TADs if we were to close that upper teeth gap.
A few other treatment options were discussed and in the end, she decided to proceed with composite veneers.
It is important to discuss treatment options with the patient and go through the pros and cons with them. I always allow my patients to think about their treatment options for a few weeks prior finalising the treatment plan to better the outcome and expectations.☺️
