3. Borang pengesahan alamat rumah yang dibeli dibawah Lembaga Pembiayaan Perumahan Sektor Awam (LPPSA)
4. Salinan kad pengenalan pemohon (diakui sah)
5. Salinan kad pengenalan pasangan (diakui sah)
6. Salinan surat beranak anak sekiranya ada (diakui sah) - yang ni lebih baik sertakan surat beranak ye. Bukan salinan mykid.
7. Salinan sijil nikah/surat pendaftaran perkahwinan (diakui sah) - yang ni diperlukan sebab bagi yang dah berkahwin dan bujang, kadar yang akan dibayar oleh jabatan adalah berlainan.
8. Salinan surat kelulusan dari pihak LPPSA (diakui sah)
9. Salinan Perjanjian Jual Beli/ S&P (diakui sah)
10. Maklumat bank seperti akun untuk gaji
* Semua dokumen ni hanya 1 salinan sahaja setiap 1.

1. Cover letter
2. Home Transfer Elaun Claims Form (Attachment B-2)
3. Home address confirmation forms purchased under Public Sector Housing Funding Board (LPPSA)
4. Copies of applicant's introduction card (validly recognized)
5. Copy of couples introduction card (validly recognized)
6. Copies of childbirth letters if there is (recognized) - this is better to include childbirth letters. Not a copy of mykid.
7. Copies of marriage certificate / marriage registration (recognized) - this is needed because for those who are married and single, the rate that the department will pay is different.
8. Copies of graduation letter from the LPPSA party (recognized valid)
9. Copies of Buy / S & P Agreement (recognized)
10. Bank information like account for salary
* All these documents are only 1 copies each 1.
