Gotta be honest, this is one of the most challenging cases I’ve done so far. Single anterior discoloured tooth is always hard to deal with, especially with a gummy smile. But I’m excited to share!🤗
To achieve this result:
Smile Design • Gingivectomy • In-office Dental Whitening & Internal Bleaching • Mock-Up • 7 unit Composite Veneers & 1 unit Ceramic Crown
Patient has excessive gingival display (“gummy smile”) with hyperactive upper lip elevator muscle. We’ve discussed the options of:
🦷Orthodontic TX with TADs (“bone screw”)
🦷Botox injection to help reduce the upper lip elevation upon smiling
🦷…or simply accepting the profile and patient just have to learn to smile a little less wide
Patient opt for no Ortho tx nor Botox, and only the treatments I’ve mentioned above. Fully understanding the limitations of the improvement we can bring her, she trained herself to smile to her ideal height and I actually think it looks good.😊Sometimes, no treatment is also a form of treatment, don’t you agree?
Composite veneers weren’t too hard to place. However, it was difficult to mask the discoloured tooth. Root canal treatment was done many years ago and we can actually see the root discolouration showing through the gingiva. (Watch my story to learn more) Even with internal bleaching, the improvement was limited, unless we carry out soft tissue surgery to increase the thickness of the 21 buccal gingiva to mask the root discolouration.
We managed to mask some of the crown discolouration on 21 with procelain-fused-zirconia crown (to match the crown to the rest of the tooth was difficult as well) and the help of a little opaquer, yet there is still room for improvement in my opinion.
Although challenging, I’m blessed to have a very kind patient who’s willing to go through the whole treatment with me. There’s no difficult treatments, as long as our partner (aka patient) is a great one. Of course, it is cases like this which help us become better clinicians. The power of a smile!😁Not only he smiles so much more vibrant now, but looks younger too! 🤩✨ I had male patients hesitant to get smile makeovers because “men don’t need to look good like women do” but I absolutely disagree because EVERYONE deserves to have a set of nice teeth!🦷
Peg laterals combined with chromogenic stains has given a suboptimal smile to this patient. With the power of digital smile design, we did two mock-ups for him:
- Veneers modifying peg laterals
- Veneers from 4-4
Patient decided to proceed with 8-unit Emax veneers to better the smile arch as well as improve the shape of his other teeth.

The tricky part to the peg lateral prep is how far back should we extend the margins. We need to give adequate porcelain thickness for best aesthetic outcome as well as patient comfort (no one likes “winged” veneers, thanks to my ceramist’s teaching🙏), so we wrapped the margins a little pass contact point.
Since most of you have asked,I’m using EXOCAD to help with my digital smile design. As “digital” as we can go, it is still important to understand patient’s concern, as well as the essentials of a smile.

Patient had her pre-existing composite veneers done elsewhere, and requested for a change due to chipping issues & to better the appearance of the veneers.
Her composite veneers were pretty satisfactory in terms of appearance, but we decided to give her a fuller smile arch with gingivectomy & 8-unit Ceramic Crowns and Veneers.

One of the question I get the most is when to decide whether to go for crown or veneer prep when the tooth is vital. My answer? Think like a ceramist. Imagine where your margins are going to end, and whether the ceramic thickness is adequate to achieve the tooth shape desired. (I’ve learned this from one of the best ceramist!)
In this case, we’ve done Emax crowns from her 12-22 due to multiple large interproximal restorations as well significant increase in tooth length. The rest are veneers as we intend to reduce her buccal corridor, hence minimal prep is achievable.
Ceramic veneers are definitely more time & energy consuming but the outcome is definitely more aesthetic & long-lasting than composite veneers! ✨In the end of the day, when a patient is able to smile happily, it’s all that matters!
