話說救護哥哥個page分享他對不負責任的COVID病人或反疫苗的人不滿,認為他們沒有考慮過醫療系統,只在乎自己,沒有想過對社會其他人的影響。之後被瘋狂圍攻,更甚指控他其實想做少點個案才這樣說。
真心,不用對這種大放厥詞的人太上心,往往無事是講說話最大聲,覺得自己有交稅就有資格有權利盡情使用醫療服務,認為醫護well paid for it,即使工作期間感染都是”You made your own informed decision”,打得份工就預咗。
我不知道謝婉雯醫生泉下有知,仍在生的家屬們聽到自己寶貴的親人,為了治療照顧病人而不幸殉職,卻被說成是「打得份工就預咗」,到底會有何感想。大概沒想到才過了18年,竟然做醫護要立下「預咗會感染」的決心,而新一代一些市民更進化成
Oh sorry,我有交稅我要自由,I couldn’t care less about your or other people’s safety
這種心態大概解釋了為什麼每年打風暴雨都總有人要去行山看海被困,而要消防救護冒險去救人。
But you know what?有人有交稅嘛,應份享有緊急服務的權利,前線緊急服務說要「預咗」。
這些「應份」享用醫療服務的人總是覺得自己可大安旨意透過「交稅」理所當然地換取醫療服務。但醫療服務和資源是有限的,世界各地都向世人展示了當醫療系統崩潰時,影響的不單止是感染的人,其他病人如癌症、外科疾病需要手術治療、突發性疾病如中風/心肌梗塞、慢性肺阻塞/肺炎/肺栓塞等等,都會受到影響。
澳洲自爆發後已將所有non emergent and non urgent surgeries/procedures延後。這代表什麼?就是很多不會死或嚴重影響生活的疾病並無法「享有」被治療的權利,因為資源有限而COVID卻佔了大部分寶貴的ICU資源。
很多術後要去ICU的手術因為無床而要延期,例如器官切除的手術、開腦手術、心臟手術等等;一些原本應該去ICU處理的嚴重疾病因為無床而要在一般病房處理。
一些非緊急但對生活質素有影響的治療或手術要被延期,例如腎石、割膽、照腸胃鏡、換關節等等,結果就是其他病人要繼續承受疾病的徵狀影響。
就是因為有人有交稅,覺得自己應份享有醫療服務的權利,認為戴口罩或打疫苗是侵犯自由,如果不同意就是歧視,質疑為何糖尿病或痴肥的病人不需要遵守Public Health的指引或要求,他們一樣佔用醫療服務。
講得出這種說話的人對醫療系統接近零知識,糖尿病和痴肥的人並不會一夜間爆發再逼爆ICU和呼吸機和氧氣需要。在糖尿病或痴肥病人身邊的人不會因為在相處超過15分鐘而被定義為close contact而要隔離,在旁邊的人不會因此要擔心感染,感染後day 7-12會不會出現嚴重併發症或需要ICU。

你知道嗎?在爆發國家中,如果你肚痛盲腸炎去急症,剛好遇著旁邊的人是感染者,你便可能需要被隔離14天,在醫院內如果你是close contact,差不多大部分重要的檢查都會被延期,特別是入侵性的檢查或手術。你想照個transesophageal echo?你想照scopes?如果不是生與死的分別,會等到14日後clearance後再照。會不會有臨床影響?大概不會令你死,但多少延誤了治療和outcome not as optimal
你知道嗎?在爆發國家中,每日都有人因感染或醫院因太多感染者而令醫療服務延遲。急性血栓塞性中風,黃金4.5小時大家都知道,但當COVID爆發時會怎樣?就是照掃描會好麻煩,中風病人想照CT Angio 放射科要做precaution、運送感染者時要清走廊、照照完後要進行深度清潔,當中涉及的人力物力和因種種特殊需求而延遲的檢查都令一些本身可能可以有更好的outcome的病人miss個黃金window而無法接受治療。
如果中風的是你親人,因為太多人受感染而延遲照CT到最後即使有血塞都無法用溶血針,你又會不會可以大放厥詞話自己有交稅,「應份享有」醫療資源的權利?
的確,你有權享用醫療資源
But you can only access what is available
當醫療系統爆煲時,很多日常我們視為理所當然的醫療服務都會變成奢侈品。
另一個有趣的地方是,很多人會說疫苗只研究了一年多,不夠臨床數據,一般藥物需時幾年。的確,一般藥物研究時間較長,但這次疫苗的研發亦是史無前例的多資源和人口數量的數據。
的確如果你不相信疫苗可以自行決定不打,但如果你的理由是疫苗太新的話,那麼感染後為什麼又突然不擔心新藥的潛在副作用?為什麼又可以接受類固醇、新的抗病毒藥、免疫療法和monoclonal antibodies?
人總是這樣,無事無幹就話你玩弄恐懼,just a weak flu,有事就第一個跪地要用最新治療和藥物。

"You see," he explained, "I consider that a man's brain originally is like a little empty attic, and you have to stock it with such furniture as you choose. Now the skilful workman is very careful indeed as to what he takes into his brain attic.
